                                 A Medical Scribe is essentially the physician's personal assistant, doing EHR paperwork, gathering information for the patient's visit, and collaborating with the physician to provide the most efficient patient care possible.   • A scribe must also be aware of the documentation difficulties, such as whether symptoms or histories should be included or excluded from the document.
